# Configuring the State Form Activity Template

#### Configuring the State Form Activity Template

The Onboarding State Form activity type is used for displaying the Onboard State Form activity.

**About this task**

The State Form activity has been added to the Template Type drop-down list in the Manage Activity Fields screen.

To display this activity:

1. In the Global Toolbar, display the **Settings**menu and then select **Settings**.
2. Select the **System Configuration** tab, if not already selected, to display system configuration options.
3. Display the **Show**drop-down list and then select **Activity Fields.** In the Manage Activity Fields screen, Activity Names and Fields panel,
4. Select **State Form Activity** from the drop-down list. The Activities State Form Activity screen now displays shown in the following figures.

The Onboarding State Form task displays in Onboard (for new hires). For example, a Pennsylvania state tax form.
