Review a Req Addendum Form

When a req is declined or canceled, a req addendum form might appear to be completed with the reason for declining or canceling the req.

Process

Review a Req Addendum Form

1. Select Menu → Reqs → View my reqs → Declined, or Menu → Reqs → View all reqs → Declined.

2. Select the req number.

3. Select the History tab.

    

4. Select View addendum.

5. Review the form.

6. Select Close to close the form.
